Electronic Applications

Electronic applications are intended to assist your application for services without coming to ECCCM in person.

Please note: You must take personal responsibility for your application.

Pay close attention to ALL instructions, ALL requirements, and upload ALL required documents.

Your application will be reviewed and if it is not complete, it will NOT be forwarded to case management for the followup phone call interview.

The success of YOUR application is YOUR responsibility.

COVID-19 Relief Fund

The COVID Relief Program is for verified COVID-19 related situations such as job loss or layoff, illness, quarantine and such. You must be able to prove a COVID-related situation.

REQUIRED DOCUMENTS to be uploaded by you:

  • Drivers License
  • Income verification (last 30 days is required)
  • Letter of furlough or job layoff due to COVID-19
  • Explanation of benefits for SSDI and other disability payments
  • Most recent pay stubs or earnings record
  • Alimony and/or child support checks
  • Section 8 Annual Certification Letter
  • A copy of ALL CURRENT BILLS, EXPENSES and INCIDENTALS. You must show all expenses, not just the bill for which you are requesting assistance.
  • A current bank statement showing the last thirty days of activity.
  • Mortgage information if requesting mortgage assistance.
  • Current lease and Landlord information, as required for ECCCM’s approved vendor list, if requesting rent assistance.

Emergency Crisis Financial Assistance

A crisis is a one-time exceptional situation that should not be repeated. Assistance is available for mortgage and rent payments, utility bills, pharmaceuticals, and fuel oil or kerosene purchases.

REQUIRED DOCUMENTS to be uploaded by you:

  • Photo ID and SS numbers for each family member, green cards, or other positive identification.
  • Proof of all income (as applicable) and/or benefit award letters
  • Most recent paycheck stub, retirement earnings, SS, or other earned income
  • Explanation of benefits for SSDI and other disability payments
  • Alimony and/or child support checks
  • Section 8 Annual Certification Letter
  • A copy of ALL CURRENT BILLS, EXPENSES and INCIDENTALS. You must show all expenses, not just the bill for which you are requesting assistance.

Hunger Relief Programs

Hunger Relief is available to households that are experiencing food insecurity and struggle to provide adequate quantities of food for their families.

REQUIRED DOCUMENTS to be uploaded by you:

  • Photo ID and SS numbers for each family member, green cards, or other positive identification.

You will also need to know how many persons are in your household, your gross income, and who will be picking up your food.

Student Hunger Prevention Program

The Student Hunger Prevention Program targets at-risk school children, and their families, with adequate supplies of nutritional food and child-friendly snacks. The goal of this program is to help the child’s developing brain with nutrition while combatting childhood obesity with appropriate snacks.

REQUIRED DOCUMENTS to be uploaded by you:

  • Photo ID and SS numbers for each family member, green cards, or other positive identification.

You will also need to know how many persons are in your household, your gross income, and who will be picking up your food. You will also have to list the school(s) that your child(ren) attend.

General Assistance Fund

The General Assistance Fund is designed to help qualifying applicants receive assistance with deposits. In order to qualify, applicants must follow the following guidelines.

REQUIRED DOCUMENTS to be uploaded by you:

    • Photo ID and SS numbers for each family member, green cards, or other positive identification.
    • Proof of all income (as applicable) and/or benefit award letters
    • Most recent paycheck stub, retirement earnings, SS, or other earned income
    • Explanation of benefits for SSDI and other disability payments

    • Section 8 Annual Certification Letter

    CIP and LIEAP Assistance

    The CIP and LIEAP funds are designed to help households who qualify according to state guidelines for poverty. In order to qualify, applicants must follow state guidelines.

    ELIGIBILITY GUIDELINES

    Household must meet ALL of these factors:

    • Effective October 1, 2019, proof of income is required;
    • Household must meet income limits for the program;
    • Household must be responsible for its heating bills;
    • Household cannot have resources over $2,250.00;
    • Household must include a U.S. Citizen or an eligible alien; and
    • You must apply in the county in which you reside.

    BASIC INFORMATION NEEDED

    1. Information about your household’s income. If anyone works – bring all wage stubs for the month prior to the month you come to apply;
    2. Information about your household’s bank account (savings or checking accounts);
    3. Information about your household’s property, stocks, bonds, and other assets;
    4. Name, date of birth, and social security numbers, of each household member;
    5. Picture ID;
    6. Copy of utility bill.
